Requirements

  • ViroPharma utilizes an independent medical education agreement for all approved grants
  • ViroPharma will not provide funding, or be responsible for funding any grant request until: (1) a complete grant application has been properly submitted; (2) the application has been approved by the Educational Grants Committee; (3) an agreement between ViroPharma and the provider, in a form acceptable to ViroPharma, has been executed.
  • Grant requestors must complete the ViroPharma budget template included with the application
  • The educational grant requestor is solely responsible for and has full control over the content of the activity
  • ViroPharma's support of the program must be clearly acknowledged and disclosed to all participants in all grant activities and materials
  • Grants must be used exclusively to fund specific educational programs. Grant requestor's overhead and general operating expenses will not be reimbursed under the grant program
  • Grants that offer promotional exhibit or display space to all commercial supporters as part of their overall funding of an educational program are permitted, but any portion of the grant intended to pay for promotional exhibit space will be denied and referred to the appropriate sales or marketing organization for separate consideration
List of necessary documents required to complete the Grant Request form:
  1. Grant request letter, which should include the following:
    • Proposed date of the program
    • Topic for the program
    • Proposed speaker(s) (if appropriate)
    • Proposed level of support
    • Approximate number of attendees
    • Provide a detailed breakdown of how the requested funds will be allocated
    • Full agenda, if applicable
  2. ViroPharma letter of agreement    Download Form (Word)
  3. Detailed, overall budget for entire program
  4. Requesting institution's Policy for Commercial Support of CME
  5. One of the following Federal Government IDs for your institution:
    • Form W-9 with Tax ID #
    • Form W-9 Tax Exempt #
